SPOTS ARE LIMITED AND PRIORITY WILL BE GIVEN TO THOSE WHO DO NOT HAVE THE ABILITY TO WORSHIP.
Please note that filling out this form does not guarantee that you will be allow. 
Please note that you can only sign up for the next Sunday. Submissions will run Monday - Thursday. Any submissions outside of that window will be voided. No advance submissions will be accepted. 

Thanks for submitting! Jan will call those who can attend on Friday. As spots are limited and priority will be given to those who can not virtually worship there is no guarantee that you will be selected.
